We’re grateful for your business, and our Rewards program is our way of saying thank you. Every time you shop with us, you’ll earn points that can be redeemed for savings on future in-store purchases.

You’ll receive 10 points for every pre-tax dollar spent. We also run special customer appreciation events throughout the year, giving you additional opportunities to earn bonus points. Once your balance reaches 2,500 points, you can redeem your first reward.

Important: Completing the form below enrolls you in the program, but you’ll still need to visit the store to collect your physical Rewards card.

Submitting the form lets us create your Guardian Rewards account so you can begin earning points right away. Please stop by the store afterward to pick up your card.

    1. The Guardian® rewards program is independently operated by each participating Guardian® pharmacy location, and the card is only valid at the pharmacy listed on the reverse.
    2. Each participating Guardian® pharmacy has the right to revoke card privileges at any time at their discretion.
    3. Cardholders may earn points on any eligible purchase as determined by each participating Guardian® pharmacy location. Some exclusions may apply. Ask an employee for complete details.

    5. Points are only redeemable at the Guardian® pharmacy location(s) indicated on the card.
